Closed-loop phase-locked rTMS treatment decreases global cortical excitability in major depressive disorder patients

چکیده

Abstract Repetitive transcranial magnetic stimulation (rTMS) has been shown to be an effective treatment for major depressive disorder (MDD). Although considerable clinical data have supported its efficacy, exact mechanism of action and optimal combination parameters remains unclear. Recent research suggests that the delivery rTMS causes long-term inhibition excitation neurons in varied brain areas. Additionally, interest steadily grown using neurophysiologic measures cortical excitability as a biomarker outcomes MDD. Under hypothesis EEG alpha phase is index across or within network, our group developed novel closed-loop system where pulse train can triggered at specific each patient's prefrontal rhythm optimize outcomes. Twenty-four MDD patients were randomly assigned either synchronized unsynchronized beginning treatment. Changes global mean field power used characterize response administration. During six-week daily period, we observed changes quasi-alpha band (6-13 Hz) rest are related both groups. Notably, received showed higher decreases after session compared group. More importantly, group, weeks with greater decrease Hamilton Rating Scale Depression significant paired (before vs. treatment) decreases. Our findings provide additional evidence rTMS, serve reliable Moreover, phase-locking induce reduction excitability, thus might help increase efficacy. The Psychomotor Retardation, a Marker of Response to rTMS Treatment in Patients with Major Depressive Disorder?

Now more than ever, let us continue to develop personalized medicine to improve the remission rate for treatment resistant depressed patients. Recent findings about psychomotor retardation a core symptom of depression provide new insights into the predictive efficiency of personalized treatments, such as rTMS.

Evolving Refractory Major Depressive Disorder Diagnostic and Treatment Paradigms: Toward Closed-Loop Therapeutics

Current antidepressant therapies do not effectively control or cure depressive symptoms. Pharmaceutical therapies altogether fail to address an estimated 4 million Americans who suffer from a recurrent and severe treatment-resistant form of depression known as refractory major depressive disorder. Subjective diagnostic schemes, differing manifestations of the disorder, and antidepressant treatm...

MRI-guided dmPFC-rTMS as a Treatment for Treatment-resistant Major Depressive Disorder.

Here we outline the protocol for magnetic resonance imaging (MRI) guided repetitive transcranial magnetic stimulation (rTMS) to the dorsal medial prefrontal cortex (dmPFC) in patients with major depressive disorder (MDD). Technicians used a neuronavigation system to process patient MRIs to generate a 3-dimensional head model. The head model was subsequently used to identify patient-specific sti...

Vortioxetine versus placebo for treatment of major depressive disorder

Introduction: Major depressive disorder is a common mental condition associated with substantial morbidity and economic burden. Approved by the FDA in September 2013 for treatment of episodes of major depressive disorder, Vortioxetine is one of the newer options available in this important area of therapeutics. Materials and methods: A comprehensive literature search (PubMed, the Cochrane libr...

Non-linear EEG analyses predict non-response to rTMS treatment in major depressive disorder.

OBJECTIVE Several linear electroencephalographic (EEG) measures at baseline have been demonstrated to be associated with treatment outcome after antidepressant treatment. In this study we investigated the added value of non-linear EEG metrics in the alpha band in predicting treatment outcome to repetitive transcranial magnetic stimulation (rTMS). METHODS Subjects were 90 patients with major d...
